libxl: support usb controller hotplug

Support USB controller hot-plug and hot-unplug.

 #virsh attach-device dom usbctrl.xml
 #virsh detach-device dom usbctrl.xml
 usbctrl.xml example:
 <controller type='usb' index='0' model='qusb2'>
