CHANGELOG.md: Use "xenbits.xenproject.org" in links

Compared to "xenbits.xen.org", "xenbits.xenproject.org" appeared
later as a name, with the intention of becoming the canonical one.
Therefore, this commit unifies all the links to use "xenproject"
in the links.
